Home Care Program Assistants (hybrid schedule) - Hiring Bonus!

Multifaceted social service agency serving elderly and persons with disabilities, seeks Home Care Program Assistants to provide high level customer service while assisting with administrative and consumer related functions to support the work of ESWA case management staff and aids in the mission of providing high quality supports and services to ESWA consumers enrolled in the State Home Care Program. Completes administrative functions as assigned by Supervisor. Telephonic outreach to consumers, documentation of findings in collaboration with and support of the Case Manager including vendor calls and coordination. Assist Case Manager in maintaining the consumer’s care plan and following up on suspended consumers and any associated data entry. Telephonic outreach to community partners to assist identifying current resources available to ESWA consumers through direct or indirect referrals. Completes and maintains up to date documentation in case records as required by the Executive Office of Elder Affairs (EOEA) and ESWA.

Job Requirements

Home Care Program Assistants (hybrid schedule) - Hiring Bonus!

Associate’s Degree and/or bilingual and 1-year relevant experience preferred. High School diploma with 2 years relevant experience may be substituted for degree. Desire and commitment to work in a Social Service agency. Excellent written, verbal, and interpersonal communication skills required. Computer proficiency, including experience with Microsoft Office products (Word, Excel, Outlook, SharePoint) required. Ability to keep accurate records, document clearly, and to communicate effectively verbally and in writing all within a fast-paced environment required. Ability to work in an interdisciplinary model. Use of a reliable vehicle including active vehicle insurance with a current license to drive required.

 - A hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year. The bonus for this position is $1,000.
